What do I do when my intake form is not mapping or creating a new lead?

You are probably using the 'Contact information' intake form type so that the submission would be saved under Contacts, not Leads.

For creating new leads directly, consider using the 'Lead Information' intake form question type, which is ideally suited for this purpose.
