How to Collect and Organize Client Information Using Lawcus Intake Forms

A quick overview of the available Client Intake Form question types in Lawcus and how each can be used.

Lawcus Client Intake Forms include specialized question types that allow you to collect and organize information about clients, leads, and related contacts. When the form is submitted, the information collected through these question types can be used to create or update the corresponding records in Lawcus.

This article explains how to add and configure Client Information, Lead Information, and Related Contact Information question types in a Client Intake Form.


Adding a Client Question Type to an Intake Form

Follow these steps to add one of these question types to an Intake Form:

  1. Navigate to Automations from the left sidebar.

  2. Select Intake Forms.

  3. Open the Intake Form you want to edit.

  4. Click Add Question.

  5. Select one of the following question types:

    • Client Information

    • Lead Information

    • Related Contact Information

  6. Click Customize on the right side of the question.

  7. Select the information you want to collect.

  8. Mark any fields that must be completed as Required.

  9. Save your changes.

Understanding the Question Types

Client Information

The Client Information question type collects information about the primary client completing the Intake Form and creates/updates the contact.

You can customize the question to collect details such as contact information and other available client fields. Once the form is submitted, the information can be used to create or populate the client's profile in Lawcus.

Example: Use Client Information when sending an intake form directly to a new client who needs to provide their personal and contact details.

Lead Information

The Lead Information question type collects information about a potential client during the intake process.

The submitted information can be used to create or populate the corresponding Lead record in Lawcus.

Example: Use Lead Information when an individual is completing an initial intake form before they become a client.

Related Contact Information

The Related Contact Information question type allows you to collect information about another person connected to the client or matter.

A related contact could be a:

  • Spouse

  • Family member

  • Paralegal

  • Agency representative

  • Business representative

  • Other person acting on behalf of the client

The submitted information can then be added as a Related Contact in Lawcus.

Example: If a client is providing information about their spouse as part of a family law intake, you can use Related Contact Information to collect the spouse's details and associate them with the appropriate record.


Customize the Information You Collect

After adding one of these question types, click Customize to select the specific fields you want to include in the Intake Form.


You can also mark individual fields as Required. For example, you may require a client's email address and phone number to ensure the necessary contact information is collected before the form can be submitted.

Only select the fields that are relevant to your intake process. Keeping the form focused can make it easier for clients to complete.
