+ Add New Formin the Form Builder, and provide aTitlefor the form.Use the
Addbutton to include questions, and ensure the question type isSingle Choicefor conditional logic.Locate the desired question & answer, choose
Show/Hide questionsfrom theEllipsismenu, and configure the visibility of subsequent questions based on the answer.Select the answer option and check the checkbox for the next question that should appear based on the answer.
