  1. Click the Automation on the left sidebar

  2. Click on Add

  3. Fill in title, description and add your questions.

Add a Title and Description

The first step is to add a title and description for your client intake form.

Add Your Questions

On the new form, the system added a default multiple choice question for you. You can change the question type by clicking on the drop-down menu that says “Multiple Choice.” You can then choose to add more questions by clicking on the 'Add' button below the question.

Adding Pages

Pages allow you to separate your intake questions into sections. You can add as many Pages as you’d like to a single client intake form or add conditional logic flow to set up skip logic processes. The “Add page” button is found on the menu that is displayed by clicking on the 'Add' button below the question.
